Job Description:
A company is looking for a Care Manager who plans and manages
behavioral and/or physical care for members.
Key Responsibilities:
Advocates for and educates members on services and self-management
techniques
Conducts assessments and develops care plans aligned with treatment
goals
Coordinates care among various providers and documents all
interactions
Required Qualifications:
NYS RN or LCSW/LMSW (any state) for Medical Care Management
3 years of experience in a mental/behavioral health or addictions
setting for Behavioral Health Care Management
1 year of pediatric clinical field experience for pediatric
positions
Licensure as LMFT, LMHC, LPC, or licensed psychologist (any state)
for Behavioral Health roles
Relevant experience as a Care Manager is preferred
