  • Collaborates with clinical case management staff to develop policies and procedures to address high risk clinical needs of Select Health members. Determines best interventions to serve the HIV population.
  • Creates assessment and reporting tools to ensure consistent evaluation and tracking of Select Health members. Oversees monthly reporting of progress; analyzes results and makes recommendations for changes, as appropriate.
  • Trains staff on identifying adherence barriers and intervention opportunities and on engaging Select Health members regarding follow up care.
  • Monitors and oversees vendors performing outreach and adherence activities and PCP sites serving the HIV population.
  • Evaluates the quality and appropriateness of services provided by VNS Health Select Health; identifies opportunities for improvement and provides recommendations to leadership.
  • Organizes and chairs case conferences between the plan and partners.QualificationsLicenses and Certifications:
    • License and current registration to practice as a Registered Professional Nurse in New York State required
    • New York State License and current registration to practice as a Nurse Practitioner in New York State preferredEducation:
      • Bachelor's Degree in Nursing or a health care related field required
      • Master's Degree in business, health care administration, or management preferredWork Experience:
        • Minimum of five years of progressive health care management experience, including a minimum of three years of experience (working with HIV patients with a focus on public health) required
        • Strong knowledge of health care delivery systems, managed care, and patient care required
