St. Vincent's Medical Center in Bridgeport has more than 3,200 employees. It includes a 473-bed community teaching hospital, a 76-bed inpatient psychiatric facility in Westport, a large multispecialty provider group, and special needs services for adults and children. St. Vincent's is the first hospital in Fairfield County to be integrated into the Hartford HealthCare network, and is the system's second-largest hospital.

The Navigator is a member of the healthcare team who helps patients with resource needs, coordinates follow-up appointments as needed, and helps reduce the number of return visits to the ED.
--- - -Responsible for identifying and resolving issues that affect the safe delivery of patient care transitions from the emergency department to the community setting. -
--- - -Provides education to help patients understand their diagnosis and treatment options.
--- - -Provides education about and assistance with connecting services available throughout our system.
--- - -Additional responsibilities include the completion of culture results reconciliation and ED patient callbacks.
--- - -Serves as a resource to staff and external contacts on issues related to patient care, patient flow, and clinical standards, policies and procedures. -
--- - -Is a role model for teamwork, leadership and professionalism in the Emergency Department.
Qualifications


Licensure / Certification / Registration:
--- Required Credential(s):
--- Licensed Registered Nurse credentialed from the Connecticut Department of
- -Public Health obtained prior to hire date or job transfer date.
--- One or more of the following:
--- Certified BLS Provider credentialed from the American Heart Association
- -(AHA) obtained prior to hire date or job transfer date.
--- Certified BLS Provider credentialed from the American Red Cross obtained
- -prior to hire date or job transfer date.
--- Preferred Credential(s):
--- Certified Certification specializing in Oncology credentialed from the Oncology
- -Nursing Certification Corporation (ONCC).
Education:
--- BSN Required.
Work Experience:
--- 2 years ED experience required
