Administrator, you will work towards enhancing care program
effectiveness through continuous assessments, patient engagement,
and coordination with healthcare teams. The position involves
managing performance metrics, ensuring compliance with industry
standards, and overseeing both onshore and offshore
teams.Responsibilities:
- Identify areas for improvement and implement strategies to
enhance program effectiveness.
- Conduct regular assessments to ensure alignment with industry
best practices.
- Coordinate patient enrollment in care management programs.
- Schedule patients to care coordinators, ensuring an equitable
distribution of patients daily, weekly, and monthly among the team
members.
- Collaborate with healthcare providers, including physicians,
nurses, and care teams, to ensure seamless care coordination for
patients enrolled in PCM and CCM programs.
- Generate reports and analyze data related to program outcomes,
patient satisfaction, and adherence to quality standards.
- Assess the need for an offshore team to support program
activities.
- Provide training, guidance, and supervision to ensure the
offshore team's productivity and adherence to program goals.
- Foster a collaborative and positive work environment to promote
productivity and job satisfaction.
- Collaborate with leadership to establish monthly targets and
performance indicators for PCM and CCM programs.
- Regularly review and report on program performance against set
targets.
- Develop action plans to address any performance gaps and ensure
targets are met or exceeded.
- Ensure that PCM and CCM programs comply with all relevant
regulations and industry standards.
- Implement quality assurance measures to guarantee the delivery
of high-quality services to patients.
- Identify areas for program improvement and develop strategies
to enhance the quality and effectiveness of PCM and CCM
services.
- Participate in quality improvement initiatives and monitor
program performance metrics.
- Prepare and deliver monthly CCM reports to the facility,
including accurate POC names and email addresses, to ensure the
database is up-to-date and aligned with communication needs.
- Maintain effective communication with direct supervisors and
leadership, providing regular updates on program progress.
- Act as a point of contact for any inquiries or concerns related
to PCM and CCM programs.Qualifications:
- Previous experience in care program management, patient
engagement, or care coordination, preferably in PCM and CCM
programs.
- Previous experience in a leadership role or
administrative.
- Experience in data analysis, reporting, and developing
performance metrics to drive program improvement.Salary Range: $29
