Practice Medical DirectorLead the strategic transformation toward

- Provide comprehensive clinical care for patients with multiple
comorbidities including CKD primarily via telehealth with
occasional in-home visits.
- Monday - Friday 8 am-5pm schedule with voluntary on-call
coverage to 10 pm through Monogram Health First clinical
program.

risk stratification, care coordination, and patient engagement

- Utilize data analytics and performance metrics to track and

- Treat patients as the direct provider in collaboration with
APPs and interdisciplinary team, providing evidence-based care
including complex care through pharmacological and
non-pharmacological interventions.
- Lead the development and implementation of clinical guidelines,
protocols, and best practices to standardize care delivery and
promote evidence-based medicine.
- Collaborate with executive leadership, clinical teams, and
external stakeholders to develop and implement value-based care
strategies that improve patient outcomes, reduce costs, and enhance
the overall quality of care.Position Requirements
- Medical degree from an accredited medical school.
- Board certification in Internal medicine, geriatrics, or family
medicine.
- Active medical license and DEA registration in at least one
state and willing to get licensed in multiple states in the
region.
- Minimum of 3 years of clinical practice experience, with
demonstrated leadership and management experience in healthcare
delivery or administration.
- Excellent analytical skills, with the ability to interpret
data, analyze trends, and make data-driven decisions.
- Exceptional communication and interpersonal skills, with the
ability to effectively engage and influence stakeholders at all
levels of the organization.
- Demonstrated ability to lead change, drive innovation, and
implement complex healthcare initiatives in a dynamic and
