Summary: Provide weekend home health physical therapy services to patients in Philadelphia, PA, using clinical skills and a compassionate approach to promote comfort, independence, and dignity in the comfort of their own home.Key Responsibilities:

• See patients through their care needs from hospital discharge to living a safe home life.
• Perform diagnostic tests and measurements, including mobility, strength, and balance assessments.
• Develop and implement individualized care plans, including manual therapeutic exercises and interventions.
• Continually assess and revise care plans, participating in multidisciplinary care planning as needed.Requirements:
  • Active and current license to practice as a professional Physical Therapist in PA.
  • Experience with OASIS preferred.
  • Able to travel to cases as assigned.
